I'm jumping into this thread a bit late so I don't know if this has been covered, but are you specifying the encoding in your form? If you leave it out, your browser assumes this: <form action="foo.php" method="post" enctype="application/x-www-form-urlencoded"> However, if you're selecting a file to upload via an <input type="file"> then you should change your encoding to "multipart/form-data".
