Point 1 is make sure you have put "ismap" in the image tag (I'm sure you have)

Point 2 is you have to either know all about your server or, yeah, "check with your 
server administration!"

Apache should know what to do with an NCSA format .map file.  With other servers they 
have to be set up by the server admin so that a program like mapcgi processes the map 
request.  I don't think there were even any Windows servers back when these standards 
were set up so it might be something entirely different on an IIE system.  I think you 
are in the hands of whatever tech support is available from the hosting company, I do 
hope they are as good as mine and actually interested in resolving this kind of issue!
