I use the Yale Style Manual
http://info.med.yale.edu/caim/manual/contents.html as well. I use 595 pixels
wide as the max width.

They recommend 295 pixels for height (and their examples show that this is
correct.) I don't worry about that for most pages, since people know to
scroll. But opening pages, esp. navigational table of contents frames,
should fit within 300 pixels.

Since I work on 19" monitor set at 1024x768, I made up a page that has a
horizontal and vertical bar. It's actually a table with the height and width
(no graphics, it opens faster.)

This creates a table cell that is 595 pixels wide, 295 pixels tall. I then
resize a browser window to that size.

This table creates a thin line that I can use to size my browser to the
right width.

I use the gold color because it can be left for quite some time during the
development and it looks good; others think it's some sort of decoration.
