My thoughts/observations:
1st page: has a stray option button at left side near bottom
2nd page: looks/functions fine. Recommend you check width with various
browsers to make sure it fits inside 640 pixel width.
3rd page: recommend you check to see if all prompt and name were filled in.
4th page: rather than automatically emailing result, allow user to view/test
it, then trigger an email when they're ready. Submit button shows text
entered on previous page, plus "Build form now".
5th page: testing. Got these errors: You did not enter the required First
You did not enter the required Last. None of the fields I name were "First"
or "Last". Click on "go back to fresh form" takes me to "Your finished form,
sans any input boxes" (probably just not ready yet, eh?)
Please keep us posted on your progress! Will be nice if you can add other
input types...and maybe allow user to define what happens to accumulated
data (ie, how it's formatted or where it's sent). Giving user the option to
include ENV variables when/if the form results are saved would be nice.
