I have installed an Information Request Form on my clients site.
He wants to verify that certain fields of data are filled out and
otherwise reject the submittal with a "missing info" comment if they
aren't filled out.

Just one suggestion -- make sure they are *positive* they *have* to
have the fields involved and that they have a good reason for
wanting them.
As a Web professional outside the US, it is highly frustrating to
meet a form which demands that I enter a state or a zip code. Far
too many forms do this while at the same time they acknowledge
that there are other countries by having them in a drop-down list!
Phone numbers are another thing which people often make
compulsory for no particularly good reason (specially seeing you
have the chance to capture their email instead). I've been rung too
many times at 3am by people who think we're in California
somewhere...I now list my fax number if they insist on the phone
field being filled in.
Another trap for unwary players, though not half so common, is the
form which requires a timezone in GMT format that won't accept
GMT +12 or +13. I've had a number of Webmasters accuse me of
not living on this planet because I've put this in and their form barfs.
OK, so you only alienate a few Kiwis, but why limit it unnecessarily
