I would suggest that when the slide show is downloaded it opens in a new tab. When I finished reading I was tempted to close the window to return to the app. I would also suggest a short description on each information request screen to help the user with what is being requested.
on the continuing education section it asks * Explation of "Other" - but i dont see where I would have been able to choose Other. * Highst Degree you have earned?. - Gives a checkbox rather than text I did not check all of it but hope this helps a bit.
