Yes, I'm required to login and am logged in. I found the problem and the fix is to insert this line:
console.append(add) at at line 1927 of sqlhtml.py. The problem is what I mentioned in the earlier post. If you override the provided search form with your own, the add button is not added to the console portion of the page. Adding the line above fixes that.
