Code to enable reading of MathML via assistive technologies should go right into WebKit itself - that's where we have the support for reading paragraphs or tables. It doesn't have to be an extension. WebCore/accessibility would be the place to look. In particular that's where code to read math via VoiceOver on Mac, Orca on Linux, or tools like JAWS or Windows-Eyes on Windows. I'm not sure if that's the sort of thing you have in mind.
