A note in the X098 IG on Page 303 for the Service Facility 2310D Loop states:
5. The purpose of this loop is to identify specifically where the service was rendered. In cases where it was rendered at the patient's home, do not use this loop. In that case, the place of service code in CLM05- 1 should indicate that the service occurred in the patient's home. My question is: Why doesn't a similar statement exist in the X096 IG for the Service Facility 2310E Loop? Is a home address required for Institutional Home Health claims? If yes, then where must it be reported? Or is a Facility Type Code for Home Health in Loop 2300 CLM05-1 sufficient for UB as well as HCFA?
