I have no idea.
There are 3 places to identify the Sender and Receiver with an ID (ISA, GS, and NM1).  As far as I know, the specs don't specify any relationship between them.
 
I know that the THIN clearinghouse uses the same numbers in all 3 places.  This, of course, makes it imposiible for me to determine which of the identical numbers is being used to identify me to them, and to route transactions from me to their destinations.
 
From observation, the numbers are proprietary (mine is our login ID), although the payer ID number seems to be based on a national plan identifier plus a code for payer type.
 
Since the number seems to be proprietary, it needs to be established by TPA, just like those #!??@  'ZZ' codes in the ISA. 
 
So, I guess that the best definition is "A sequence of characters that identifies the Sender or Receiver to the processor of the transaction".  Can anybody out there come up with a better one?

Indeed, who really uses the 1000A and 1000B loops in the 837 (the
submitter and receiver) at all?  How are the ETIN and the 1000 loops
used?  Is the ETIN a proprietary number?  Or does it have some relation
to the ISA receiver or sender field? Why does it have to be agreed upon
in a trading partner agreement?
