Follow-up Comment #4:

I've now observed this at the beginning of THoT::The Underlevels.  The bug
occurs when the AI is firing in response to a player missile attack.  The
sound effect and animation play twice, but the unit is not actually firing
twice; we can tell this because he doesn't inflict damage.

To reproduce, load the attached savefile and do missile attacks (with Lords
and White Mage) on the enemy Thunderguards.  Once in a while (not always)
you'll get the double-shot effect

Hmmm...Cathy, re-testing in the other room, says she just saw it happen
during an AI turn.

I think suokko is right and this is some sort of animation engine problem.
