Follow-up Comment #3, bug #19419 (project wesnoth):
There's also the following possibility:
A mage attacking an orcish assassin with a ranged attack could get poisoned,
but wound the assassin enough for a second unit to finish it off, which in
turn could expose other wounded units...
There's also Regeneration. For a unit that can regenerate poison is not as
much of an issue.
If the goal is to kill the target getting poisoned is a minor concern...
In short, avoiding poison is not always a concern for the player or the AI.
