Make a webpage that gets a known value from the database.  Set up an
http monitor on that webpage.  Have whatsup scream if the webpage says
anything other than whatever it's returning from the database.  No fuss,
no muss, and you don't have to worry about port 3306.

You're not selecting the right option.  When you click "new", make sure
you
change the service type to "SNMP Threshold" rather than TCP/IP Services.
Then
you can enter the MIB and the value to monitor.
