Try changing the source address to an actual account on the server, like
your e-mail address for instance (at least use a valied domain name).
Some mail servers check that.  

Also, try temporarily removing all of the % signs and junk in the
notification when you test it, some SMTP servers don't like all that
junk in there.  Setup a notification with very simple things in the
subject and message body.  No %'s or variables of any kind...when an
actual notification is sent, there will be test in these fields, but for
the test, it litterally sends all of the %'s...

ah - I'd keep your notification process as simple as possible.  You have
two links in your notification chain that can break.

I also DO NOT use anything but the direct IP address of the mail server
delivering the mail.  (example: [EMAIL PROTECTED])  No chance for
a DNS failure to stop your mail from getting through.
