The "Insert media" button, in the pop-up box, is not in a good place, from UX
perspective. It should be in the top right of the box, not the bottom right. It
would be even more clear to editors what the button did if (a) it was greyed
out until a thumbnail is selected; (b) it had a "Cancel" button to its left;
and (maybe) (c) it was labeled "Save image". [The cancel button duplicates the
"X" for closing the box, of course, but this is actually good UX design -
giving users multiple choices for performing the same action, as long as that
doesn't clutter things up.)
