Please go to Preferences / Editing and select 'Enable enhanced editing toolbar' and unselect 'Show edit toolbar'. I use the Vector skin and the problem was tested without any user defined javascript and CSS. It was also confirmed by Andre Klapper in Bug 61220. This post is almost a duplicate of 61220 and is only created because I couldn't change Bug 61220 to indicate that it's a Proofread page.
