I have seen this happen on occasion usually after an SP update. During the install the user will be asked to provide the original MS cd.

The user may not have the cd handy so continue the install without it. After the SP update is done when trying to access a given application then the user will be asked for the cd repeatedly. The system is still looking for verification of the original cd.

Usually re-installing the SP update properly will solve the problem.
