Hi,
I fixed both ends, which were the same, 1 thru 8 to 1 thru 8 instead of 1&2
to 1&2, 3&6 to 3&6, 4&5 to 4&5, 7&8 to 7&8, which is I suppose why the
"tester" didn't see a problem!
Yesterday, I made a mistake in a cable 3&4 swapped at one end), & the tester
found that immediately (visual inspection also could have!).

Did you check or repair both ends?
"Sometimes" cables do not "have to be" "standard",
as long as "the mistake" was made on both ends...
I have seen cross-connected optical feeds "correct"
themselves by having a double cross-connect...
(Slightly different I know, but the same general idea...)
