I would suggest that the card reader may be faulty.
I don't know what camera program you use.
But I use a canon and the pics are actually in a separate folder inside
the main canon folder. So they are not readily apparent at first glance.

I have never had a problem in anyway and have all my pics stored on my
hard drive. From there I retrieve them and name them using Thumbs
Plus. Delete the bad ones.
