when i  did it in  the  past  this  was  the  way  I  did it, furst i  set up  
the  headings like  you have  done  then  I  went  to  tables,  then create 
table in  the  tables menu,  then  there  was  an option  called  I beleave 
make  content table  then  it  give  you   a  wizared for the  contents table  
where you  can  chose how  you  want  it.  
infeary  wance you deside on  your lay out  and  whether you want hyperlinks or 
not for the  headings to let  you jump to  each heading.
you  should just  be able  to click finesh    I think it is.
then you should have  the table  made  for you.
Well thats  how its  ment  to  go lol   it neaver normaly  worked out quite 
like  that  for  me there  was  all ways some think that  shouldent  be in  
there that was lol
Its one of  the things  that seems  to  bemore fidaly  than it  should be.  I 
found  with it  that trile and era seemed to  work the best.
dont know if this helps or not.  It  has been  quite  some  time since I  made  
a content table for  a document.
