To consider the question seriously, the Federal gov. is the one entity that represents 
all of us (albiet not equally) and has the ability to create a long term solution.  It 
would necessarily not be "efficient" or quick because of the need to represent many 
needs but it has the only real chance of success. I recall reading speculation several 
years ago that if every worker in the country contributed to a universal, single payer 
system, the premium for each worker would be $25 per month. 

I would send Walmart to Iraq to rebuild the country with super-stores and parking lots 
and give everyone a job with enough income to shop happily at Walmart.
