I've attached the two custom authentication samples that Cisco sent to me. I haven't compared them line by line, but they might be very similar.

I see that someone has already posted the return codes, but the above files also include them.

We did something similar, but using LDAP. I agree, the lack of
documentation
was very frustrating. At one point, in order to make the enter/return
key
work, I changed the form field type of the login button to submit from
whatever it was (or the other way around) and things went haywire. Now
users
must click on login button in order to login, they can't press the
Return
key. Granted, I'm sure we can spend some time figuring it out, but we're
not
to big on the HTML in our group. ;)

Anyways, when we called for support, it was like, um, there is no
support
for that, you're on your own. We had to push and push and push some more
in
order to get the return codes and what they all meant. After a few times
of
"it's up to you to program", we had to escalate the case in order to get
a
text file of the sample return codes.

We're using a customized screen with the "webauth" feature on the WLC
to
authenticate users against a RADIUS server.  I found the lack of
documentation (at the time) frustrating - we could chat off-line if
you
need some pointers.
