MicroSemi (PowerDsine) 7000 series or 9000 series
The 9000 is 802.3at compliant (30+ Watts).
If you select models with "M" in the reference those are SNMP manageable
The 9000 can also be powered by DC (e.g., if you have UPS in the closet)

We have those all over campus and like them a lot.
Our first generation of powerDsine lasted 8 years with 1 unit that failed
over 120 midpsan deployed.
