Finally got rid of all of them last year (we also had them for VoIP phones in some places).
1) Rock solid reliable, with the exception of one that burned out the power supply completely. 2) We used the PowerDsine 9001G. Rather large for an injector, but space usually wasn't an issue. 3) Yes. Our wireless system (Juniper wireless with Ringmaster control software) does AP availability monitoring and notification.
