The Gigamon G-TAP TAP-251 is an OM3 multimode passive tap that works extremely well, and is quite inexpensive and low profile. There are other models for different grades of fiber including OS1/OS2 single mode. You'll likely also want the tap chassis which is a 0.5U rackmount that can fit 4 of those tap modules.
