This doesn't help you out immediately, but I've actually been working on 
web-based management software (PHP/MySQL) for quite some time that 
handles this and much more (like was suggested here, its done using 
freeradius).  Besides controlling access to your Mikrotik routers for 
management, it also has:

* Top level leads / accounts management (client details, address, 
contacts, service locations, etc)
* PPPoE account management
* Dialup account management
* IP provisioning for CPE
* Real-Time SNMP/HTTP polling of Mikrotik, Canopy, CB3, Ubiquiti NS/PS  
access points and client devices.
* Real-Time SNMP polling of Mikrotik routers
* Real-time comparison of provisioning data (per-AP based) to active 
connections (shows you who is up, down, or is an unknown connection)
* A basic ticketing system for towers and accounts
* Hierarchical management of network assets (Towers, Routers, Access 
Points, etc).
* Per-tower photo and file storage

And more.

Not fully decided, but will likely be releasing the code under GPLv3 
license once I have the code cleaned up some, and some additional 
features buttoned up.  I'm still developing heavily on it to meet my own 
desired rich-feature set.  Some slightly outdated screenshots that at
