OK I created the form as a message in outlook and sent a test. If you send it and open it in another outlook it looks fine you see the form. But any other browser all you see is a message and the form is a .dat attachment with no way to read it.

To I have to do something else Do I have to have some scripting code in their.  What I want is the form to come back as HTML within a email so if viewed in HTML compliant email client it looks like an HTML form and can be filled in and sent back.

Isn’t that how it works?

Dan,
One way to do this is with Adobe Acrobat (full product). You can send an adobe form as an email attachment, it can be filled out and the actions set to post to the URL of your choice. The user can not change the fields in the Adobe form. Additional the user can make a nice printed copy of his form.
