I use the Survey system from Fergal/Witango.

After making some modifications [Multiple Choices, Radio Buttons, Text Areas, Multiple languages, some bug fixes ;-)] I must say, it works very well.

The chamber of commerce use it and if somebody like to use it, I can publish a link for the download.

But take care: It have multiple languages (English, German), but after adding some new features, some variables exist only in German - so you must translate it (the language is based on a external XML File).

the measly $200 bills: date-range for which survey will run, require
unique ip addresses/cookie/etc, customize css templates (question/answer
styles), import/export survey data and a ton more

equivalent and I don't mind paying a similar amout for a
similarly-featured program; I can't write a survey system this robust for
$200 bills.

as I really need a survey tool with these features.  It may have an Oracle
or MySQL database backend.
