You will no doubt hear from Ben on this list in favor of MDaemon. I will add my two cents, also in favor of MDaemon, together with Security Plus.
My email address is thoroughly polluted, in that it was harvested from several hundred domain records and scores of other places in the bad old days. Unfiltered, my account is a write-off, and would receive something in excess of 1,000 spam a day. MDaemon together with Security Plus doesn't blink, removing all but just the occasional email, meaning one every two or three days, with virtually zero false positives. I administer it with my eyes closed for tons of users. I don't let them store huge amounts of mail on the server, and I don't let them accumulate mail there forever. I give them a 100-day limit, beyond which old messages are removed. It's a simple five out of five rating from me.
