One way would be to set the 'hidden' letterhead and footer to

display:none;

in the screen media css, and to remove display:none; from the print style sheet so they appear when printed.

note this means they will be visible in source code and to search engine spiders

Is it possible to create a 'hidden' letterhead and footer that would only show up in printed documents from a website?

If so, what would be the best-practice markup-wise given the semantic implications of hiding this content (company name/contact details/physical address/legal notice/etc.) from screen view?
