Why aren't book, magazine, and newspaper publishers "required" to

Old media... but how difficult would it be? I'm sure it could be done!

Why aren't TV broadcasters and movie production companies "required"
to sub-title all of their broadcasts or films, or have an "off screen
reader" describing the scenes?

Because nobody uses their TV or the local cinema to access government
services or buy goods. On the web you have these government and
business websites which are more comparable to physical public and
business spaces rather than visual media.
