[WSG] Accessible likert scale (disagree/agree/strongly agree/etc) forms

Hello All,

I'm working on a Likert scale questionnaire (Strongly Agree/Agree/ 
Undecided/Disagree/Strongly Disagree) with 20 questions and some  
Googling came up with the following approach...
