We’ve had the same problem on FT8 at W4DUG, the Tampa ARC setup. We’re running 
remote through TeamViewer with WSJT-X installed on the local computer, so there 
are no DACs to the client computer. Had it happen with several operators; we 
just restart the program. Sometimes at restart, the audio levels are way too 
low; we just restart again…………….

Seems to be when it's in a Tx/Rx cycle but not sure about that.

It's not the WAV file causing it as a replay works just fine.
