XercesStdOutputWriterException

Hello, I was wondering if anyone could shed a little light on the following exception that is thrown from my transformer->transform() function call. XercesStdOutputStreamWriteException: Error writing to standard stream!.  The error code was 12. (, line -1, column -1) I tried to look for information on this exception class, but couldn't really find anything that would tell me what an error code 12 means.
