In each case, there is some variation on <xsl:attribute name="prefix:localpart" namespace=""> The first three cases use the prefixed name without emitting a namespace-decl for the prefix; the err19 case has an illegal xmlns:prefix="" declaration. Xalan-J raises an error for these cases, but other prominent processors are more willing to change the prefix or omit it to get the attribute in the correct namespace. The spec isn't really clear about whether this is a discretionary choice.
