In these cases, you have: (1) a declaration of a different default namespace at the stylesheet level, (2) xsl:attribute has a namespace attribute set to the same URI, and the attribute name is unprefixed in 128 and prefixed in 130. The attribute comes out with a leading colon (no prefix), and there is no namespace declaration in the element's start tag. By contrast, if the namespace URI is different (127 and 131), the attribute is issued with properly synchronized prefix and declaration.
