document() function is used to get references to another XML document
(from within your XSLT stylesheet). It doesn't do the job of
transformation. The logic of transformation is written within your
stylesheet.

Regarding your question about CDATA - anything written within CDATA
section is treated as a normal text node. You cannot treat something
written in CDATA section as markup.
