This has come up before (e.g. http://sourceforge.net/p/xbiblio/mailman/message/31972432/). citeproc-json definitely needs an extension mechanism.
The question, of course, is how to keep it from becoming another mess, like with the proliferation of MarkDown formats, for example. Maybe a GitHub repo where folks could register their own extensions would be a good idea.
