You may need different grips on your poles. There are a number of good ergonomic grips available for different brand poles. Some of these can be installed on existing poles. Think about how often you move your hands and arms during the marathon. Overuse injuries are common for some people in our sport.
I have always gone to a sports doctor for my sports injuries. The basic RICE method works the best for taking care of the injuries. R est I ce C ompression E levation And Ibuprofin. I don't want to recommend a dosage, but I usually take 3-200mg tablets 3 times a day if the injury is extra sore. I make sure that I take food with the tablets. Your tendonitis or sore knuckles are probably due to an inflamation from overuse.
