Hello everyone,

I inherited an xCat 2.16.3 system after the previous sysadmin left nor with any 
handover notes.

I recently upgraded some nodes from RHEL6 to RHEL7 x86_64 via xCat without any 
issues on the cluster.

After my users tested stuff out the past month and there were no screams, I 
went to update the remainder of the nodes by installing them with RHEL7 via the 
xCat system just as before.

The installs seemed to go fine as last time, the nodes PXE-booted, caught the 
boot image, installed stuff, then rebooted, did more updates and then rebooted 
again.

It was here that all the updated nodes in this batch are failing and dropping 
to the dracut emergency shell. I checked (tabdump'd around) all the node 
settings, lsdef, etc. and nothing has changed since the first batch of nodes 
were successfully updated previously.

It appears the /dev/mapper/xcatvg-root went missing, the actual member 
partition of the vg (/dev/sda4) is present with the TYPE=LVM2_member 
declaration but just isn't associated with xcatvg-root (no other vg's present 
either). I'm sure I can fix it with a rescue media boot, create the xcatvg-root 
vg and add /dev/sda4 to it, but I'm at a loss why to understand why my xCat 
system worked fine previously updating nodes last month and now it suddenly 
doesn't. All the server hardware/firmware involved are identical. Any ideas 
where I should be looking?
