I now finished most of the changes I needed to make to xcircuit for adding 
cairo.
The cairo rendering takes care of anti-aliased drawing as well as proper font 
support.
Due to the changes, also the X11 way of drawing has changed.

At present there should be no differences between X11 and cairo (apart from the 
looks).
I might (will) have introduced more bugs into xcircuit. Especially colouring 
and updating might be of in corner use cases.

The cairo rendering can be enabled by adding --with-cairo to the ./configure
To see proper font rendering the ghostscript fonts (URW) should be installed. 
As you are using xcircuit, I assume ghostscript is already installed on your 
system. If not, install the fonts. On my OpenSuse system the package is called: 
ghostscript-fonts-std.
