Alternative to SAX->buffer->pull is a "throttled" SAX stream, as used in
Xalan's IncrementalSAXSource adapter. We also have a version which
leverates Xerces' incremental mode, but we wanted to be able to run
parse-on-demand with other SAX sources. Downside is that this requires
some multitasking ("coroutines").
