This does not correspond to a known bug. This does not happen in our own
documents and test cases.
1) Make sure that the layout of your pages is adapted to the headers and
footers you need to generate. That is, double-check the dimensions in:

2) Do not let our tool process *indented* XSL-FO files (indented XSL-FO
files are only useful when debugging). In some cases, our tool thinks
that open lines are significant and generates empty paragraphs for them,
hence extra empty pages.
