Maybe specifying the bus ID in the device sections helps, i.e.

It's coincidence that anything is displayed on the external head without
that option and on the LCD with it. The only way to get mirroring with
4.2 is to set up multihead with overlapping screens. The driver in
current CVS (to become 4.3.0) OTOH should mirror by default, and
multihead may work better as well.
