re-appearing inside the instance...

Sixten, could you provide us with the first few lines (xml & doctype
declaration, some comments) of your misbehaving document?

Kimbro, Tom & folks, is this a serialization problem? People get to
rewrite the Xerces serializer cos' it's no good...
