It can also happen with some of the brain-dead address verification schemes 
out there. They connect to you and run a SMTP session up to the RCPT TO: 
command, and if they don't get an error, they assume the address is valid. 
These kinds of things usually (but not always) have a null sender address 
(MAIL FROM: <>).
