My included file uses a doctype bookinfo and I've tried the file with the toc tags removed. I thought that maybe it needed the TOC tag so that a toc could be generated. I've been reading docbook documentation and according to them I'm entering everything correctly. I have version .20.5 of Apache FOP installed. Was wondering if I need to try .92 instead.
