I have not tried to find why. What you have done seems to use 
the right approach except that it is possible to make it simpler.

I have quickly prepared a ``prototype'' of what will be available in XXE
V2.2. See attachment. Note that I had to slightly modify the
configuration file of DocBook 4.1.2 to make this work. I also added an
entry for DocBook 4.2 to catalog.xml.

To install this file, unzip it inside <xxe_distrib_dir> in order to
patch <xxe_distrib_dir>/config.

Pitfall:

The Article and Book templates works OK in both docbook 4.1.2 and 4.2.

The "Chapter (part of a master document)" and "Section  (part of a
master document)" templates (which have no document type declaration)
works but I cannot predict if they will be opened as DocBook 4.1.2 or
DocBook 4.2.

For these DTD-less document templates, the detect clause holds true for
both DocBook 4.1.2 or DocBook 4.2 and XXE use the configuration which
has been loaded first. This configuration may be DocBook 4.1.2 *or*
DocBook 4.2.

XXE V2.2 will be modified in order to load its configurations in
alphabetic
order but currently, for XXE V2.1, this is not the case.

PS: I'm very curious to know why you are so interested by DocBook 4.2 
(i.e. what is missing in 4.1.2, which is already a ``monster'' ?)
