Over the weekend I experimented and eventually found the same solution, so
thank you for corroborating :) It's always nice to know when one is on the
right track.

Is it possible for me to change some aspect of XXE so that, for example,
underline will be visible on screen (for something like, say, emphasis with
role)? I tend to include references to menu options inline in text, and
since many are underscored (to indicate the Alt key combination) I try to
duplicate the menu option format in my text. I will be altering/inserting
the relevant <fo:> for simplified DocBook so that underline will appear in
PDFs (for now, RTF, HTML and other formats are of secondary importance) but
it would be nice if I could see that on-screen too.
