Thank you for your response. 
In the meantime I have found that the problem is caused by the question 
marks. If I replace the "???" or delete the question marks the file 
chooser dialog, which is diplayed after a double-click on the image 
placeholder, works as expected. There must have been a change from 3.2.0 
to 4.1.0, because we have used this fileref suggestion string from the 
beginning.

By the way: 
Is it possible to write a macro command that allows dragging an image in 
the clipboard and dropping it on the image placeholder? Such a command 
should only ask for the filename in order to save the image and to set the 
fileref-attribute of the imagedata element.
